HOSPICE SERVICES:
Hospice offers comprehensive physical, psychological and spiritual care for each patient and their loved ones. To qualify for Hospice services a patient must have a terminal diagnosis, and a limited life expectancy (6 months or less), which is determined by a physician. The primary focus is on comfort measures when all “curative” efforts have been exhausted.
SERVICES OFFERED:
Our Hospice team includes skilled nurses, occupational, physical and speech therapists, chaplaincy, social work, nursing assistants and volunteers. Our compassionate care team is available for consultation and supportive services 24 hours a day/ 7 days a week. Oversight of each individualized care plan is provided by our Hospice Physician or a Nurse Practitioner based on the patient and family’s needs. Services take place in the patient’s home or residence, such as an assisted living facility or nursing home. Hospice is a comprehensive program with bereavement services available following the death of a loved one.
PAYMENT OF SERVICES:
Hospice is covered primarily through Medicare. It may also be covered through Medicaid and some private insurance companies.
HOME CARE SERVICES (also referred to as HOME HEALTH):
Home health services may be needed for individuals who are recovering from an illness following a hospital or facility stay, or who need additional support to remain safely at home. The primary care physician makes the referral for rehabilitation to take place in the residence (for example, a nursing facility, assisted living or home). A nurse will make a home visit for a complete assessment of needs. Following this initial assessment, the nurse will discuss an appropriate schedule for follow up visits which may include physical, occupational, and/ or speech therapy and will formulate a personal care plan specific to his/her needs.
SERVICES OFFERED:
Services that may be provided are nursing support, physical, occupational and speech therapy, certified nursing assistants and medical social workers. The goal is to help the patient recover from an illness or surgery and to become as independent as possible. The care team along with the patient will help create an individualized plan of care.
PAYMENT OF SERVICES:
Home health services are paid for through Medicare, Medicaid and private insurance companies. A nurse will verify the insurance coverage prior to the initial visit.
