Hospice and Palliative Care of the Blue Ridge has been established since 1992. We are proud to serve Yancey, Mitchell, Avery and upper Burke and upper McDowell Counties. We are a non profit agency governed by a Community Board of Directors made up of the communities we serve. Our mission is to provide exceptional, compassionate care to individuals facing a life limiting illness regardless of ability to pay. Our first priority is our patients and our community. We believe in giving our patients and their family members the support and education needed to understand their illness. Our goal is to provide care that is unique to each individual. Our care allows our patients and their families to "Make Each Moment Count". We provide 24 hour availability of nurses, pain and symptom management, social work support , in home aides, as well as, chaplain and volunteer services. We care for patients in their own home, as well as, nursing homes, assisted living and family care homes. We cover all related medications, medical equipment, supplies, treatments, as well as related hospitalization and respite care.
We offer a Palliative Care program directed by a very experienced staff. Our Pathways Palliative Care Team offers in home visits to patient's in their home, nursing home, etc. to offer symptom management, education/emotional support, and case management.
We also offer bereavement services to anyone in our service area who has experienced loss. The goal of the bereavement program is to provide opportunity to develop a better understanding of grief, learn coping skills, and share feelings. It is not necessary to be a family member of a hospice patient to participate in our bereavement program. Our regularly scheduled support group meeting is open to anyone.
